Concussion/Neuro-Optometric patients with an Initial Consultation booked should complete both forms before their first appointment.

Melbourne (Yarraville) Patients:
Is this your, or your child’s first visit? To make your appointment as smooth as possible, please remember to bring:
Your child’s Medicare card.
Any glasses or contact lenses (even if they’re broken, or unworn.)
Relevant reports or referrals from teachers, OTs, or Paediatricians (etc).
Geelong Patients:
Once you have booked your Initial Consultation, please complete the relevant pre-exam form (for a child, or an adult).

Billing & Cancellations
Kiddies Eye Care is a private billing practice, which means that while Medicare rebates apply to many of our services, full payment is required on the day of consultation, which includes a gap payment. Our helpful clinic team will process your Medicare rebate on the spot (if applicable), with relevant rebates being paid to the account you have nominated through Services Australia.
Late cancellations (within 48 hours notice) or no shows, will incur a $100 if your appointment cannot be filled. Should you need to cancel, please let us know ASAP to provide us with the opportunity to fill your appointment and remove the fee. We understand that sometimes kids pick up viruses; if you are unwell and have cold/flu symptoms or you need to cancel please call and leave us a message (24 hours).
Different consultation types have different fee structures; if you have questions about what to expect to pay on the day of your consultation please contact us. Please note that there is a weekend surcharge for Saturday appointments.
